I. The coverage of student group insurance for Academic Year 2022 and 2025 is provided by Mercuries Life Insurance Co., Ltd.

III. Length of claim application validity: 2 years (Please file the insurance claim as soon as possible with all required documents for fear of missing the claim deadline.)
IV. Documents to be submitted:
A. Medical Reimbursement:
1. A diagnosis certificate(s) issued by the hospital(s)
2. Receipts for relevant medical expenses
*Either originals or photocopies of the diagnosis certificate(s) and the receipts are acceptable. The hospital seal should be affixed if a photocopy is provided.
3. A traffic accident registration form (required in case of traffic accidents)
4. A photocopy of both sides of the student ID card
5. A photocopy of the student’s passbook (the front side only)
6. Students under the age of 18 must attach a copy of their parent’s (guardian’s) ID card.
7. International students (currently enrolled) are required to provide a photocopy of the Alien Resident Certificate or the Entry & Exit Permit
8. An X-ray film/disk (the original copy) for those with a bone fracture
9. A pathology report for those applying for cancer insurance claims for the first time.
10. Applicants for hospitalization claims related to mental illness must additionally submit nursing reports for the hospitalization period.
B. Death benefits
1. A death certificate or a public prosecutor autopsy report
2. A household registration transcript after the registration cancellation for the deceased is completed
3. Beneficiary (Legal Heirs): A copy of the household registration transcript, a copy of both sides of the ID card, and a copy of the front page of the bank passbook. (Both parents must prepare these documents.)
